How to write a letter of intention to return to a program to repeat a course?

Ok, I've took this cl but you have to score a 90% or above and only one attempts, didn't realize that I couldn't make-up for the test again, I got dropped out of the program, in one condition i could come back, is to write an appeal letter, I wrote my letter, i just need some idea, one of the main reason I didn't success academically in that course was the driving, too far I had to drive 45 minutes to one hour, it really kills me, second I have test anxiety, burnout, nervous, nauseous, whenever I'm taking a test, don't know why but it happened to me, and some personal issues
